When applying for a child passport, you need to submit the following documents and meet these requirements:
Provide a certified official U.S. Birth Certificate (Texas and California “Abstract” birth certificates are not acceptable). If the child has a valid passport, include it in addition to the birth certificate. Alternatively, provide the original U.S. Naturalization Certificate, FS-240, DS-1350, or Certificate of Citizenship Abroad.
Provide a clear color copy of both parents' valid ID (Driver’s License, State ID Card, Military ID, or Passport). If a parent’s name has changed, include the original marriage certificate, divorce decree, or court-issued name change document.
Submit two new passport photos (taken within the last 3 months). The photo must not be stapled to the application (the acceptance agent will staple it). Ensure that no hat or glasses are worn in the passport photo.
Fill out the Letter of Authorization form by hand with blue or black ink. Only check the first two boxes at the top, do not check the third box. The Courier Company Name at the bottom must read “Urgent Passport”. If applying for a child passport, one parent must sign their own signature in the "Applicant Signature" section.
Complete the DS-11 form online. Print the completed form with the barcode. It must be typed and not handwritten. The fee should include $100 for the passport book, $60 for expedited service, and $21.36 for overnight delivery.
Include one of the following proof of travel: a printout of your airline ticket email confirmation with your name as the passenger, a foreign hotel confirmation with a letter explaining ground travel, or a company letter for business travel.
If one parent is not present at the Acceptance Agent, the non-present parent must provide a notarized consent form along with a copy of their valid ID.
If the child passport applicant does not have an SSN, provide the "No Social Security Number" form. This form is required by the State Department for all passport applicants.
